If you have never had a nametag, you will receive it for free. If you need to replace a lost nametag, we ask for a donation of $10 to cover the cost of the nametag. Please use the giving portal below after you submit the form. Under fund, select other payments. In the memo line, please write nametag.

Students in 5th grade and above are welcome to order a nametag.
